Dear Class of 2028,
This week's message, from Yale Student Health, explains how to start the health forms you will need to provide in late June, signed by your health care provider, and stresses the importance of making medical appointments now because they take time to schedule.
You will also want to read about the health care coverage in which you are automatically enrolled, and how to waive it if you have alternative coverage. You are required to have health insurance to cover hospitalization and medications.
Before closing with an introduction of the services available to you, the Student Health team urges you, as we do, to talk with your parents and guardians this summer about the transition ahead and what it means for managing your own health care, particularly if you have chronic or specific health care needs.

As you make medical appointments, remember that Camp Yale forms, also signed by a health care provider, are due July 15. In addition to the risk management waiver, which is required of all programs, FOOT and Harvest have their own additional medical forms, so submit them, too, by July 15 if you are considering those programs among your top three.
Dear Class of 2028,
Congratulations and a warm welcome from Yale Student Health. We are excited to welcome you to campus! Below is a brief overview of services offered at Yale Health and links to our website.
First, we need to draw your attention to two critical steps you need to take now to ensure your necessary medical forms are completed and your medical insurance is in place before you arrive on campus.
STEP 1
COMPLETE YOUR REQUIRED HEALTH FORMS

Download the Student Health Requirements Packet. This includes detailed instructions, a checklist and key forms that your current primary care provider must complete and sign:
• Immunization Form
• TB Risk Assessment Form
• Physical and Health History Form
• Consent to Treat (only if you will be under age 18 when classes begin)
Make an appointment with your provider NOW to get these forms completed. It will likely take time to get an appointment so don’t delay!
Upload your forms to Health On Track, Yale’s health requirement portal, starting on June 26. We will notify you when the portal is open. All forms are due by August 1.

UNDERSTAND AND DECIDE ON YOUR STUDENT HEALTH CARE COVERAGE

Yale Health Basic Services. All students attending Yale at least half-time and working toward a Yale degree are automatically eligible for Yale Health Basic Services. You do not need to sign up or pay an additional fee for these services. Basic Services includes visits to Student Health, Athletic Medicine (varsity athletes), Mental Health & Counseling, Acute Care, Gynecology, Inpatient Care (4th floor infirmary), and Blood Draw. Also covered are emergency contraception and nutrition counseling. Yale Health Basic Services is not considered a health insurance plan.
Yale Health Hospitalization/Specialty Care coverage. All students attending Yale at least half-time and working toward a Yale degree are required to have health insurance to cover hospitalization and medications. Students are automatically enrolled in and charged a fee for
Yale Health Hospitalization/Specialty Care coverage. This includes all the services in Basic Student Health Services, plus a wide range of Specialty Services, Obstetrics, Physical Therapy, Hospital and Emergency Services, Eye Care, Diagnostic Imaging, and Pharmacy, and on-site infusion services.
Alternate insurance (e.g. your family's plan). Students who have alternate insurance can waive Yale Health Hospitalization/Specialty Care Coverage. If you are unsure whether to waive Yale Health Hospitalization/Specialty Care coverage for this upcoming semester, fill out our Waiver Checklist which will help to walk you through your options. The deadline to waive for the Fall term or full year is September 15th. The deadline to waive for the Spring term is January 31st.
College is a time of great transition and managing your own healthcare for the first time might be new for many of you. We are here to help. We also encourage you to take some time this summer to speak with your parents and guardians about what it will mean to take charge of your own healthcare and how you can make a smooth transition to Yale Health. This discussion is especially important if you have a chronic medical condition or specific healthcare needs. Yale has great healthcare resources:
Yale Health is a multi-specialty group practice and HMO owned by Yale University. Care is provided at our state-of-the-art facility, Yale Health Center, walkable from most parts of campus. It offers a wide variety of on-site healthcare services including primary care, specialty care, acute care, mental health & counseling, radiology, blood draw, pharmacy, and a 17-bed inpatient care unit.
Student Health operates as a department within Yale Health. At Student Health, students have on-site access to primary care provided by doctors, nurse practitioners, and physician’s assistants; these primary care providers coordinate all aspects of students’ (or their dependents’) care and are ready to assist you. Telehealth, email and scheduling through our patient portal (MyChart) is available, as well as 24/7 access to nurse assistance and advice are available. You’ll hear more about these services over the summer.
